WORLD LORE - So, in the world of omegaverse, there is primary and secondary gender/sex
Primary gender is simple, it's what you got at birth, so male or female
Secondary gender is either alpha or omega, this emerges after puberty usually from ages 14-20 or so, this is often referred to as "presenting." If you were a female, after presenting as an alpha, you will grow an alpha cock, explained later you cannot get pregnant. If you were a male, after presenting as a alpha, your cock will change into an alpha cock. If you were a female and presented as an omega, nothing externally really changed. If you were a male and presented as an omega, depending on what smut you read either your current hole will function as a vagina or another hole will form functioning like a vagina, you can now become pregnant.
All alphas have alpha cocks which have knots. They look like canine penises, so the knot will swell during intercourse and after insertion will swell, locking them to their partner between 5-30 minutes. They can go into something called "rut" for a few days every month where they just get really really horny.
All omegas have a hole which can get them pregnant. They can go into something called "heat" at which point they become VEEEEERY hyper focused on sex to the extend of being nearly physically debilitated. You can take medical suppressants to help with this, but the heat will still last for up to a week. If an alpha knots you during the heat, it's calming and it'll only last for a few days with them involved.
All alphas and omegas give off pheromone smell where usually omegas smell sweet to alphas from their scent gland. If an alpha wishes to "claim" an omega and mate with them, they'll bite down on the omegas scent gland on their neck. During ruts and especially heats, the pheromone smell is extremely strong but can also usually be blocked partially with medical suppressants.
